Changing everyday habits can improve the quality of your life and reduce the possibility of illness and injury.  The following list of statements can help you to identify some important steps toward a healthier and safer life.  Mark each box next to the statement that represents your actions.  Unmarked boxes identify areas that put you at risk.

Stress

Personal Safety

17. I keep my vehicle in good operating condition.

18. I obey traffic laws.

19. I wear safety belts whenever I operate or ride in an automobile.

20. I keep recreational equipment in good working condition.

21. I wear life jackets (personal flotation devices - PFD's) when taking part in water activities, such as boating, fishing, and water-skiing.

22. I swim only when others are present.

Home Safety

23. I post local emergency number(s) near my telephone(s).

24. I have battery-operated smoke detectors where I live.

25. I keep medications safely and securely stored out of reach of children.

26. I keep cleansers and other poisonous materials safely and securely stored.

27. I turn off the oven and other appliances after use.

28. I keep a working fire extinguisher in my home.

29. I have an emergency plan in the event of injury, sudden illness, or natural disaster.

30. I practice emergency plans with my family or roommates.

but you must keep up the good work.

persevere and you will succeed.
